Practice : AI Lifecycle Governance

Purpose and Strategic Importance

AI systems are not static. They degrade, drift, and become misaligned with their original purpose as the world around them changes. A model trained on last year's data may produce subtly — or dramatically — wrong outputs this year. An AI system that was appropriate for its original deployment context may become inappropriate when the user base, regulatory environment, or business strategy changes. Lifecycle governance is the discipline of ensuring that AI systems remain safe, effective, and fit for purpose throughout their operational life and are retired responsibly when they are not.

Without lifecycle governance, organisations accumulate AI debt: systems running in production that nobody fully owns, whose risk profiles have drifted, and whose retirement is indefinitely deferred because nobody has a mandate or process for decommissioning them. This is a safety and compliance liability that compounds over time.


Description of the Practice

  • Defines governance checkpoints at each major stage of the AI lifecycle — ideation, development, deployment, ongoing operation, and retirement — with explicit criteria and accountabilities at each stage.
  • Maintains a comprehensive AI inventory that records every AI system in operation, its owner, its risk tier, its last review date, and its retirement plan or expected lifespan.
  • Establishes a lifecycle review cadence that ensures no AI system operates indefinitely without active revalidation of its fitness for purpose.
  • Defines decommissioning criteria and processes so that retirement of AI systems is a planned, governed activity rather than an organisational afterthought.
  • Ensures that lifecycle decisions — including changes in scope, deployment context, or user population — trigger re-evaluation of risk assessments and compliance status.

How to Practise It (Playbook)

1. Getting Started

  • Create an AI inventory by cataloguing all AI systems currently in operation, capturing ownership, purpose, deployment context, risk tier, and last review date.
  • Define lifecycle stages and the governance requirements at each stage, starting with a lightweight but complete model that can be refined over time.
  • Assign clear lifecycle owners for every AI system — someone accountable for ensuring governance activities are completed and for escalating when they are not.
  • Identify AI systems that are past their expected lifespan or last review date and schedule immediate reviews to bring them into governance.

2. Scaling and Maturing

  • Build lifecycle governance into project management tooling so that governance milestones are tracked alongside delivery milestones, not managed separately.
  • Develop automated alerts that flag when AI systems are approaching review dates, when performance thresholds are breached, or when compliance mapping is out of date.
  • Create a retirement playbook that guides teams through the safe decommissioning of AI systems — including communication to users, data handling, and dependency mapping.
  • Report on lifecycle governance health at a portfolio level, enabling leadership to see which systems are compliant, which are at risk, and where remediation investment is needed.

3. Team Behaviours to Encourage

  • Treat AI systems as products with lifecycles, not projects with end dates — normalising the expectation that governance is an ongoing responsibility, not a delivery milestone.
  • Build decommissioning planning into the inception of every AI system, so retirement is considered from the outset rather than avoided until crisis.
  • Flag when a system is operating outside its original scope or in a changed context, triggering lifecycle review rather than silently extending deployment.
  • Contribute to the AI inventory actively and honestly — accurate, complete records are a precondition for effective governance.

4. Watch Out For…

  • AI inventories that are maintained only at inception and never updated, creating a false sense of control while the actual portfolio drifts into opacity.
  • Governance processes that focus exclusively on pre-deployment and ignore the ongoing operational phase, where most real-world AI incidents occur.
  • Systems that remain in production indefinitely because no team has the mandate, resources, or appetite to retire them, accumulating risk without accountability.
  • Lifecycle governance treated as an overhead by delivery teams rather than as a shared professional responsibility, leading to superficial compliance.

5. Signals of Success

  • The organisation has a complete, current AI inventory with named owners and up-to-date review dates for every system in operation.
  • At least one AI system has been retired through the governance process, demonstrating that the lifecycle model operates through to completion.
  • Lifecycle reviews surface actionable findings — scope changes, performance issues, compliance gaps — that lead to concrete system improvements or retirement decisions.
  • No AI systems are operating past their scheduled review date without documented, approved rationale for extension.
  • Teams treat lifecycle governance as a normal part of their operating rhythm, not an exceptional activity triggered only by incidents.
